This section aims to provide guidance and support for Practitioners tasked with changing the way they work to help make Self-directed Support a reality.

Self-directed Support is a major culture shift in the way health and social care services are delivered. The shift sees a move towards a more person centred and outcomes focused assessment of needs and delivery of services. We have a real opportunity in Scotland- at government, professional, community and individual level- to work together to achieve our ambitions for a quality life for everyone. Every practitioner has a key role and responsibility in the process to provide people who need support with the information and assistance they need to make real choices.

Self-directed Support is about more than one particular mechanism - it is about the individual making an informed choice and deciding how much ongoing control they wish to have.
